You will also receive communication and information regarding your health via your remote health monitoring package.

These packages are designed to support people living with single or multiple long term health conditions, including Chronic Heart Failure, Chronic Obstructive Pulmonary Disease, diabetes and hypertension, to help them continue living independently and better understand their health care needs.
